We will gladly file claims with your primary and secondary insurance carrier. To facilitate this, bring your current insurance card(s) to every visit. Our staff will reconfirm this information at each appointment; however, payment of services is ultimately the responsibility of the patient. Please be prepared to pay your co-payment before each visit. Cash, checks and all major credit cards are accepted. No-show fees may be charged if you do not keep your scheduled appointment.
